§ 5-13.03. Licenses: Applications.

Any person desiring to obtain the license required by the provisions of this chapter shall make a written application to the Chief of Police therefor, stating, in addition to the general information required by the provisions of Section 3-1.124 of Article 1 of Chapter 1 of Title 3 of this Code, the following information:

(a)

The residence address of the applicant and the address of the place, if any, where such business is to be carried on;

(b)

The name under which the business is to be transacted; and

(c)

The names and addresses of all the owners and persons having an interest in the business.
