§ 4-8.04. Licenses: Required: Applications: Form.
Every applicant for a license required by the provisions of this chapter shall personally appear before the Chief of Police and make a written application to the Chief of Police therefor, stating, in addition to the information required by Section 3-1.103 of Article 1 of Chapter 1 of Title 3 of this Code relating to business licenses, the following information:
(a)
The name, age, personal address and telephone number, and principal business address and telephone number of the applicant, or if the applicant is a corporation, of its principal officers, or if the applicant is a partnership, firm, association, or other entity, of the persons comprising such entity;
(b)
Whether the applicant is applying for a license as the owner or operator of a taxicab, or both, if he is applying as both owner and operator;
(c)
A description of every taxicab which the applicant owns or operates, giving the trade, name, year, motor and serial number, State license number, seating capacity, and body style of the vehicle;
(d)
The distinctive color scheme, name, monogram, or insignia by which every taxicab owned by the applicant shall be marked;
(e)
The date place, offense, and punishment as to each prior criminal conviction of the applicant; and
(f)
Proof of negative test for controlled substances and for alcohol which complies with the requirements set forth in Government Code Section 53075.5.
